• This program (the “Program”) is available to Medtronic, Tandem, and Ypsomed insulin pump users only.
• The Program is not available to Medtronic, Tandem, and Ypsomed insulin pump users who are within their 90-day pump trial period or who have less than 6 months remaining on their current pump warranty period. Current Omnipod users are also not eligible.
• The Program allows for a Participant to receive the Omnipod 5 Automated Insulin Delivery System Starter Kit at no cost to them for a limited time.
• In order to receive the Omnipod 5 Starter Kit at no cost, Participant must register their interest and place their order for two (2) boxes of Pods between May 1st, 2026, and August 31st, 2026. However, training on the product can occur outside this timeframe.
• Pods are not included as part of this offer and Participant must purchase them separately and on an ongoing basis to keep using the Product. Participant will have to complete appropriate training and obtain the necessary authorization from their healthcare provider before starting to use Omnipod 5.
• Insulet Canada Corporation will not charge any provincial program for the cost of the Omnipod 5 Starter Kit and you agree not to seek provincial program reimbursement for the Controller received through this offer.

A 13-week randomized controlled trial conducted among 194 adults (age 18-70 years) with type 1 diabetes in France and the U.S., comparing the safety and effectiveness of the Omnipod 5 Automated Insulin Delivery (AID) System versus conventional non-AID pump therapy and CGM (control). Mean time in range (3.9-10.0 mmol/L) at 13-weeks as measured by CGM: Control = 43.8%, Omnipod 5 = 61.2%. Mean adjusted difference Omnipod 5—control = 17.5% (4.2 hours/day), P<0.0001.

• Through the Program, an eligible participant may switch to the Omnipod 5 or Omnipod DASH by paying a designated amount, as indicated herein. The Program allows a participant to purchase the Omnipod 5 Controller or Omnipod DASH PDM. Pods are not included as part of this offer and Participant must purchase them separately. Participant will have to complete appropriate training and obtain the necessary authorization from their healthcare provider before starting to use the Omnipod 5 or Omnipod DASH.
• Insulet Canada Corporation will not charge any provincial program for the cost of the Controller or PDM and you agree not to seek provincial program reimbursement for the Controller or PDM received through this offer.
